Eligibility: you must be 18 or older and meet one of the following. A BMI of 35 or above. A BMI of 30 or above with heart failure with preserved ejection fraction (HFpEF, also called diastolic heart failure), uncontrolled hypertension, or chronic kidney disease at stage 3a or later. A BMI of 27 or above with prediabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease.
Not eligible if you already receive a GLP-1 covered through your Part D plan, or if you have type 2 diabetes, moderate to severe sleep apnea, or fatty liver disease. In those cases your Part D plan may cover a GLP-1 through its own formulary instead: the Wegovy injection label carries a separate indication for noncirrhotic MASH with moderate to advanced (F2 to F3) fibrosis.
The $50 sits outside the normal Part D payment flow: no deductible applies, none of it counts toward your true out-of-pocket total, and the low-income subsidy does not apply. Medicare hosts an eligibility questionnaire at medicare.gov/glp1bridge, but that is a starting point rather than the whole path: your prescriber must submit a prior authorization to a central processing center, including an attestation that you have received lifestyle counselling. The program is time-limited, so check current terms at medicare.gov rather than any summary.
