LillyDirect Mounjaro: Flat $499 Self-Pay Price, Pen Only (2026)
LillyDirect Mounjaro self-pay costs $499 per month, and that number does not change when you move up in dose. Every strength from the 2.5mg starter through the 15mg adult maximum carries the same price, and every strength ships as a Single-Dose Prefilled Pen. There is no vial option for Mounjaro through the program, so nothing to reconstitute. This guide covers eligibility, the five pharmacy partners, how the price compares with the Lilly savings card, and how to get a prescription routed. Pricing here reflects Lilly's published program terms verified on July 28, 2026.

Key Takeaways
- Flat pricing: $499 per month for self-pay, identical at 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg, and 15mg. Titrating up does not raise your bill.
- Pen only: Mounjaro ships through LillyDirect as a Single-Dose Prefilled Pen. No vial, no bacteriostatic water, no drawing a dose into a syringe.
- Not the same as the Zepbound program. Zepbound is the weight-management brand, and its self-pay price is tiered by dose across both its pens and its vials. Those tiers do not apply to Mounjaro.
- If you are insured, price the savings card first. When a commercial plan covers Mounjaro, the card can bring a fill down to $25 for up to three months, capped at $1,950 in annual savings.
- Five pharmacy partners fill LillyDirect Mounjaro orders: Prescryptive, Amazon Pharmacy, Fuze Health, GiftHealth, and Walmart Pharmacy.
- Eligibility follows a type 2 diabetes diagnosis and a prescriber's judgment. There is no BMI threshold, because that criterion belongs to the weight-management indication.
- Map your titration and steady-state levels with the tirzepatide calculator before you commit to a dose schedule.
What Is LillyDirect and How Does It Work for Mounjaro?
LillyDirect is Eli Lilly's own direct-to-consumer pharmacy platform. Instead of shopping a prescription around retail chains and hoping for a workable cash price, you route the prescription to LillyDirect Pharmacy Solutions and pay a published, fixed price. For Mounjaro, that price is $499 per month for self-pay orders.
One distinction matters before anything else. Mounjaro is the tirzepatide brand approved for type 2 diabetes, cleared by the FDA in May 2022 as an addition to diet and exercise. Zepbound is the separate tirzepatide brand for chronic weight management. Same molecule, two brands, two indications, and two entirely separate commercial programs. This guide covers the Mounjaro path. If weight management is the reason for the prescription, the LillyDirect Zepbound guide covers that program instead, and Mounjaro vs Zepbound lays out how the two brands differ.
LillyDirect can also process insurance for Mounjaro depending on your plan's terms. The $499 figure is the self-pay lane specifically: what you pay when no benefit is applied to the order.
Mounjaro through LillyDirect is pen only, with no vial option
All six Mounjaro strengths listed on the LillyDirect page are Single-Dose Prefilled Pens, from 2.5mg/0.5mL through 15mg/0.5mL. No vial format is offered for Mounjaro through the program. That has practical consequences: no reconstitution step, no bacteriostatic water to source separately, no insulin syringe, and no drawing a measured volume. You pull off the base cap, hold the pen flat against the skin, turn the lock ring, and press the injection button. The needle is already attached and stays hidden.
This is where readers most often get tripped up. The Zepbound program does sell single-dose vials of lyophilized tirzepatide powder, and a large amount of writing about "LillyDirect tirzepatide vials" is describing Zepbound. Some of that writing, including earlier pages on this site, has extended Zepbound's vial pricing to Mounjaro. It does not transfer. As verified on Lilly's Mounjaro page on July 28, 2026, Mounjaro is a prefilled pen at one flat price.
Pharmacy partners: Prescryptive, Amazon Pharmacy, Fuze Health, GiftHealth, Walmart
Lilly does not dispense the medication itself. Five licensed pharmacy partners fill LillyDirect Mounjaro orders: Prescryptive, Amazon Pharmacy, Fuze Health, GiftHealth, and Walmart Pharmacy. The first four handle mail delivery. Walmart Pharmacy is the retail pickup route, which Lilly and Walmart launched for direct-to-consumer pricing in late 2025, and Lilly's own Mounjaro page lists it as a current partner. If you have searched for "GiftHealth Mounjaro" or "Prescryptive LillyDirect," these are the fulfillment names you keep hitting.
Both the Mounjaro and Zepbound programs run through the same five partners; which one you pick mostly affects delivery speed and whether in-store pickup is realistic where you live.

Who Is Eligible for LillyDirect Mounjaro?
Eligibility tracks the prescription, not a separate application. If a licensed prescriber writes you a valid Mounjaro prescription for type 2 diabetes, you can fill it through LillyDirect on a self-pay basis. There's no program questionnaire and no enrollment fee for the self-pay price itself.
Medical eligibility follows the FDA label
Mounjaro was approved in May 2022 to improve glycemic control in adults with type 2 diabetes, used alongside diet and exercise. The prescribing information was expanded in December 2025 to cover children aged 10 and older with type 2 diabetes, at a maximum of 10mg once weekly. Clinical support comes from the SURPASS program: in SURPASS-1, tirzepatide monotherapy produced HbA1c reductions of 1.87 to 2.07 percentage points from baseline at 40 weeks, against a 0.04 percentage-point rise on placebo.
Notice what is not on that list: a BMI cutoff. BMI thresholds of 30, or 27 with a weight-related condition, belong to the weight-management indication carried by Zepbound. Applying them to a Mounjaro prescription is a category error. Your prescriber will still review contraindications, including personal or family history of medullary thyroid carcinoma and Multiple Endocrine Neoplasia syndrome type 2, which are addressed in the Boxed Warning. See Mounjaro for type 2 diabetes for the clinical picture and what is Mounjaro for the basics.
Program restrictions worth knowing
- The $499 price applies to self-pay orders only. You cannot apply both the self-pay price and an insurance benefit to the same fill.
- LillyDirect can separately run the order through insurance, which is a different transaction with a different out-of-pocket result.
- Shipping is within the United States only.
- A valid prescription is always required. Nothing about the program removes the prescriber step.

One price, six strengths. Lilly lists every Mounjaro pen strength on LillyDirect at $499 per month for self-pay, and the format column reads the same on every row: Single-Dose Prefilled Pen. For context, the published list price for Mounjaro is $1,112.16 per fill, so the self-pay route is roughly 55 percent below list. The list figure is background here, not the number you would pay through the program.
Self-pay price by dose (as of July 28, 2026)
| Dose strength | LillyDirect self-pay | Format |
|---|---|---|
| 2.5 mg / 0.5 mL | $499/mo | Single-Dose Prefilled Pen |
| 5 mg / 0.5 mL | $499/mo | Single-Dose Prefilled Pen |
| 7.5 mg / 0.5 mL | $499/mo | Single-Dose Prefilled Pen |
| 10 mg / 0.5 mL | $499/mo | Single-Dose Prefilled Pen |
| 12.5 mg / 0.5 mL | $499/mo | Single-Dose Prefilled Pen |
| 15 mg / 0.5 mL | $499/mo | Single-Dose Prefilled Pen |
Three Mounjaro price points compared: published list price, LillyDirect self-pay, and the insured savings-card floor. Program terms verified July 28, 2026.
Correction notice: some coverage of this topic, including two older pages on this site, describes Mounjaro's LillyDirect price as tiered by dose at $299, $399, and $449. Those figures come from the Zepbound program. Mounjaro's self-pay price is flat $499 at every strength, confirmed on Lilly's Mounjaro page on July 28, 2026. Older tiered numbers on mounjaro cost and the Mounjaro savings card guide are being corrected.
Why the price does not step up by dose
Zepbound's self-pay price is tiered by dose, $299 at 2.5mg, $399 at 5mg, $449 for the upper doses with a refill inside 45 days, and it applies to both its pens and its vials. Mounjaro's is a single flat rate. Lilly publishes the two programs independently, with no stated link between them.
That is worth planning around. Tirzepatide titrates in four-week steps from 2.5mg up to a 15mg adult maximum, and it has a half-life of roughly five days, so steady state at each new dose takes about four to five weeks to settle. On a flat price, moving from 2.5mg to 15mg changes your clinical picture but not your monthly cost. The Mounjaro dosage chart lays out the full escalation path, and the Mounjaro calculator will project levels at each step.
Mounjaro Savings Card vs. LillyDirect: Which Costs Less?
Your insurance status decides this one. Three situations cover most people.
- Commercial insurance that covers Mounjaro. The Lilly savings card wins by a wide margin. It can bring a fill to as little as $25 for up to a three-month prescription, against $499 per month self-pay.
- Commercial insurance that excludes Mounjaro. The card's second tier sets $499 per fill with up to $8,411 in annual savings, which is roughly the same monthly outlay as LillyDirect self-pay. At that point convenience and pharmacy choice decide it.
- No insurance at all. The savings card requires commercial insurance enrollment, so it is off the table. LillyDirect self-pay at $499 per month is the practical route.
Savings card terms in full
Per Lilly's published Mounjaro savings terms, verified July 28, 2026. If your commercial plan covers Mounjaro:
- As little as $25 for up to a three-month prescription, if you have commercial insurance that covers Mounjaro.
- Per-fill savings maximums of $150 for a one-month fill, $300 for two months, and $450 for three months.
- A separate annual savings maximum of $1,950 per calendar year.
- A limit of 13 prescription fills per calendar year.
- The card expires and savings end on December 31, 2026.
- Anyone enrolled in Medicare, Medicaid, VA, or TRICARE is not eligible.
If your plan does not cover Mounjaro, the second tier applies instead: $499 per fill, up to $8,411 in annual savings. The $1,950 annual figure is specific to Mounjaro's with-coverage tier. Zepbound's card runs under separate terms with a $1,300 annual cap. The two programs get confused constantly, so check which brand a number is attached to before planning around it.
Medicare and Mounjaro
Here the type 2 diabetes indication helps you. Medicare Part D generally excludes drugs used for weight loss alone, which is why Zepbound coverage is so limited for Medicare enrollees. Mounjaro's diabetes indication is a standard Part D covered use. Medicare enrollees cannot use the Lilly savings card, but a Part D plan that covers Mounjaro puts you under the annual Part D out-of-pocket cap. That cap took effect at $2,000 on January 1, 2025 and is indexed each year: $2,100 in 2026 and $2,400 in 2027 (CMS and Medicare.gov, checked October 2, 2026). Compare that against $499 per month self-pay before deciding which lane to use, and check your plan's formulary tier and prior-authorization requirements first.
How to Order Mounjaro Through LillyDirect
Four steps, and only two of them are really yours to do.
Step 1: Get a type 2 diabetes prescription
Any licensed prescriber who manages type 2 diabetes can write it: primary care, endocrinology, or a telehealth clinician licensed in your state. The prescription needs to specify the Mounjaro strength and the quantity per supply, because each dose strength is its own product.
Step 2: Have the prescription routed to a partner pharmacy
Ask the prescriber to send it electronically to a LillyDirect pharmacy partner, the same way they would send anything to a retail pharmacy. Prescryptive, Amazon Pharmacy, Fuze Health, GiftHealth, and Walmart Pharmacy all appear as destinations in standard e-prescribing systems.
Step 3: Choose self-pay or insurance at checkout
Once the prescription lands, you set up an account and pick a payment path. Self-pay is the flat $499 per month. Running it through insurance instead produces whatever your plan's copay or coinsurance works out to, which may be lower or considerably higher. You choose one path per order, not both.
Step 4: Receive the pens or pick them up
Mail-order partners ship to your address. If you selected Walmart Pharmacy, you collect the prescription in store at the same price. Either way the shipment is prefilled pens, one per weekly injection, kept refrigerated until use.
The LillyDirect Mounjaro ordering path from prescription to prefilled pen delivery or Walmart pickup.
What arrives in the box
Single-dose prefilled pens, a package insert, and the medication guide. Because Mounjaro through LillyDirect is a pen product, there is no bacteriostatic water to buy, no insulin syringe to source, and no reconstitution math to get right. That removes the most common preparation error in this category. You still want alcohol swabs and a sharps container. The needle comes attached to each pen. Follow the prescribing information that ships with the medication rather than any dose volume you read online.
Frequently Asked Questions
How much does Mounjaro cost through LillyDirect?
Mounjaro through LillyDirect self-pay costs a flat $499 per month, the same at every dose strength from 2.5mg to 15mg, as published on Lilly's Mounjaro page and verified July 28, 2026. It ships as a prefilled pen, not a vial. A valid type 2 diabetes prescription is required.
What is the cheapest way to get Mounjaro?
If commercial insurance covers Mounjaro, the Lilly savings card can bring the cost to as little as $25 for up to a three-month supply, which beats every other route. Without qualifying insurance, LillyDirect's flat $499 per month self-pay price is the lowest price Lilly publishes for Mounjaro outside the savings card.
Does LillyDirect fill Mounjaro prescriptions?
Yes. Mounjaro is available through LillyDirect for self-pay customers, and LillyDirect can also process insurance depending on plan terms. Fulfillment runs through licensed pharmacy partners: Prescryptive, Amazon Pharmacy, Fuze Health, GiftHealth, and Walmart Pharmacy.
Can I get Mounjaro vials through LillyDirect like Zepbound?
No. LillyDirect offers Mounjaro only as a Single-Dose Prefilled Pen, and no vial or reconstitution option is listed for Mounjaro through the program. Zepbound, the weight-management brand of tirzepatide, does offer vials through its own separate LillyDirect program with its own pricing.
Is the LillyDirect Mounjaro price different by dose?
No. Unlike Zepbound's tiered self-pay pricing, Mounjaro's LillyDirect self-pay price is a flat $499 per month at every dose strength, from the 2.5mg starting dose through the 15mg adult maximum. Titrating upward does not increase the monthly self-pay cost.
Can I combine LillyDirect self-pay with my insurance?
No. The $499 self-pay price applies only to self-pay orders. LillyDirect can separately process an insurance claim for Mounjaro depending on your plan's coverage terms, but you cannot apply the self-pay price and an insurance benefit to the same order.
Does Medicare cover Mounjaro?
Mounjaro's type 2 diabetes indication is a standard Medicare Part D coverable use, unlike weight-loss-only drugs which Part D generally excludes. Medicare enrollees are not eligible for the Lilly savings card, but a Part D plan that covers Mounjaro is subject to the annual Part D out-of-pocket cap, which is indexed each year: $2,100 in 2026 and $2,400 in 2027. Formulary placement and prior authorization still vary by plan.
What is the annual savings cap on the Mounjaro savings card?
If you have commercial insurance that covers Mounjaro, the savings card caps annual savings at $1,950, with per-fill maximums of $150 for one month, $300 for two months, or $450 for three months, up to 13 fills per calendar year. The card expires December 31, 2026. Larger annual caps quoted online usually belong to the Zepbound card, a separate program.
Who is eligible for LillyDirect Mounjaro self-pay?
Anyone with a valid prescription for Mounjaro to treat type 2 diabetes, written by a licensed prescriber, and a US shipping address. Unlike Zepbound, there is no BMI threshold, because eligibility follows the type 2 diabetes diagnosis and the prescribing decision rather than a weight-based criterion.
How do I get my Mounjaro prescription sent to LillyDirect?
Ask your prescriber to route the prescription electronically to a LillyDirect pharmacy partner, the same way they would send it to any retail pharmacy. Prescryptive, Amazon Pharmacy, Fuze Health, GiftHealth, and Walmart Pharmacy are the listed partners. Once it arrives, you choose the self-pay or insurance path at checkout.
Do I need bacteriostatic water for LillyDirect Mounjaro?
No. Mounjaro through LillyDirect ships as a prefilled pen, so there is nothing to reconstitute. Bacteriostatic water, insulin syringes, and reconstitution math apply to powdered vial products, which is not what this program dispenses for Mounjaro.
