Foundayo (orforglipron) · Once-Daily Tablet
Foundayo Dosing: Six Strengths, One Tablet a Day
Foundayo dosing starts at 0.8 mg once a day and climbs through six tablet strengths, with at least 30 days on each dose before the next step and a ceiling of 17.2 mg once daily. This dosage chart puts every strength, its tablet, the earliest day it could start and its self-pay price in one view. Dosing facts come from the July 2026 FDA prescribing information; prices come from LillyDirect and TrumpRx.gov as of October 29, 2026. For background, read what Foundayo is and why it is not oral tirzepatide.

Key Takeaways
- Six strengths, one ladder: 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg, one tablet a day, swallowed whole, with or without food.
- 30 days is a floor, not a calendar. Each step needs at least 30 days on the current dose, and every increase past 5.5 mg is based on treatment response and tolerability.
- The label names no maintenance dose and gives no switching schedule. Both are decided with your prescriber.
- Two prices have an end date. As of October 29, 2026, the 2.5 mg ($149) and 5.5 mg ($199) self-pay prices are temporarily reduced through December 31, 2026.

*Earliest possible start is the soonest a strength could begin, not a schedule. It assumes one tablet every day with no missed doses, 30 days counted as 30 daily doses, and a step up at every first opportunity. The label sets no maximum time on any step and does not require reaching 9, 14.5 or 17.2 mg. Prices are cash prices for a 30-tablet bottle before taxes and fees, not insurance or Medicare prices.
Weighing a Different GLP-1 Pill?
The Wegovy pill is an oral semaglutide tablet with its own dose ladder and timing rules.
Read the Wegovy Pill GuideHow Foundayo Dosing Steps Up: The 30-Day Rule
The first two moves are set by the label. After at least 30 days on 0.8 mg, the dose goes to 2.5 mg; after at least 30 days on 2.5 mg, it goes to 5.5 mg. From there the wording changes. The dose may be increased to the next level (9, 14.5 or 17.2 mg) after at least 30 days on the current dose, "based on treatment response and tolerability." The maximum is 17.2 mg once daily.
The label gives no maximum time at any step and does not name a separate maintenance dose.
The six steps of the Foundayo label. Steps 1 to 3 are fixed; steps 4 to 6 depend on response and tolerability. Source: Foundayo prescribing information, Section 2.1.
The chart's earliest start days (day 31 for 2.5 mg through day 151 for 17.2 mg) come from counting 30 days per step. They are derived from Section 2.1. After 7 or more missed doses in a row, escalation restarts from a lower dose that the label does not name (Section 2.3).
Thirty days is the shortest allowed stay on a step. Nothing in the label requires reaching 17.2 mg. Increases above 5.5 mg depend on how you respond and how well you tolerate the dose you are on.
Why the Steps Exist
The label tells prescribers to follow the starting dose and escalation to reduce the risk of gastrointestinal side effects. In the two trials behind the label, nausea, vomiting and diarrhea were more common during the dose escalation period and became less common over time (Section 6.1).
Foundayo Tablets Must Not Be Split
Tablets are swallowed whole, never broken, crushed or chewed, and no more than one tablet is taken per day. So there is no label-supported way to take an amount between two strengths: a dose change means a different tablet strength.
How to Take Each Foundayo Dose
Every strength is taken the same way: one tablet once a day, whole, with or without food. Lilly's patient website adds that Foundayo can be taken any time of day, with no planning around food or drink. Tablets come in bottles of 30 with a desiccant and a child-resistant cap. Store them at room temperature, 20 to 25°C (68 to 77°F); short excursions between 15 and 30°C (59 to 86°F) are allowed (Section 16).

The colors repeat across round (0.8 to 5.5 mg) and oval (9 to 17.2 mg) tablets, so check the marking in the chart above and the strength on the pharmacy label. If a tablet does not match, ask your pharmacist before taking it.
Missed Foundayo Doses and Restarting After a Gap
| Situation | What the label says |
|---|---|
| One missed dose | Take it as soon as possible. Do not double up the next dose, and do not take 2 doses in the same day (Section 2.3; Medication Guide). |
| 7 or more doses missed in a row | Restart dose escalation at a lower dose to reduce the risk of stomach side effects (Section 2.3). The Medication Guide says to call your healthcare provider about how to restart. |
| Which lower dose to restart on | Not named in Section 2.3 or the Medication Guide. Your prescriber chooses the restart strength. |
A long gap can also raise the price of 14.5 mg and 17.2 mg; see the 45-day refill rule below.
What Changes Foundayo Dosing: Medicines and Health Conditions
Some medicines and conditions cap the Foundayo dose, change another drug's dose, or rule Foundayo out:
| Situation | Label instruction | Section |
|---|---|---|
| Strong CYP3A4 inhibitor | Foundayo maximum is 9 mg once daily. Avoid strong CYP3A4 inhibitors that also inhibit OATP1B (for example, ritonavir). | 2.2, 7.1 |
| CYP3A4 inducers | Avoid strong inducers. With moderate inducers, monitor effectiveness and escalate the dose as needed. | 2.2, 7.1 |
| Oral hormonal contraceptives | Switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase. | 7.3, 8.3 |
| Simvastatin | Do not exceed simvastatin 20 mg once daily. | 7.2 |
| Insulin or a sulfonylurea | When starting Foundayo, consider lowering the dose of the insulin or sulfonylurea (low blood sugar risk). | 5.5, 7.2 |
| Another GLP-1 receptor agonist | Using them together is not recommended. | 1 |
| Severe gastroparesis | Not recommended. | 5.3 |
| Liver impairment | Not recommended if severe (Child-Pugh C). No dose change for mild or moderate. | 8.6 |
| Kidney impairment, including end-stage | No dose change. | 8.7 |
| Pregnancy | May cause fetal harm. Stop Foundayo when pregnancy is recognized. | 8.1 |
| Breastfeeding | Not recommended for nursing women. | 8.2 |
This list is not complete; give your prescriber and pharmacist a full list of your medicines. Because Foundayo slows stomach emptying, it may also affect how other oral medicines are absorbed (Section 7.3).
What Each Foundayo Dose Did in the Label Trials
The label reports two 72-week, placebo-controlled trials, each with arms at 5.5, 9 and 17.2 mg. There was no arm for 0.8, 2.5 or 14.5 mg; those were steps on the way up, since both trials followed the Section 2.1 escalation. The trials used an investigational orforglipron formulation, which the label presents as equivalent dosages of Foundayo.
Trial 1: Adults With Obesity or Overweight, No Type 2 Diabetes
| Arm | Mean weight change, week 72 | Difference vs placebo | Lost 10% or more |
|---|---|---|---|
| Placebo | -2.1% | n/a | 13% |
| 5.5 mg | -7.4% | -5.3 points | 32.5% |
| 9 mg | -8.3% | -6.2 points | 39.8% |
| 17.2 mg | -11.1% | -9.0 points | 54.5% |

Trial 2: Adults With Type 2 Diabetes
| Arm | Mean weight change, week 72 | Difference vs placebo |
|---|---|---|
| Placebo | -2.5% | n/a |
| 5.5 mg | -5.1% | -2.6 points |
| 9 mg | -7.0% | -4.5 points |
| 17.2 mg | -9.6% | -7.1 points |

|---|---|---|---|---|
| Nausea | 10% | 26% | 34% | 35% |
| Constipation | 9% | 20% | 27% | 24% |
| Diarrhea | 11% | 21% | 23% | 25% |
| Vomiting | 4% | 13% | 21% | 24% |
| Stopped treatment for any side effect | 3% | 6% | 9% | 10% |
Source: Foundayo prescribing information, Section 6.1 (Table 1 for the four reactions; the Section 6.1 text for stopping treatment).
These are group averages, not a forecast for any one person. The label cautions that side-effect rates from one drug's trials cannot be directly compared with another drug's, so none of these tables is a head-to-head with any other GLP-1.
Why You May See 36 mg in the News
The published ATTAIN-1 paper in the New England Journal of Medicine reports doses of 6, 12 and 36 mg, with mean weight changes at week 72 of -7.5%, -8.4% and -11.2%, against -2.1% for placebo. Those milligram numbers belong to the investigational tablet used in the trial. They are not Foundayo strengths, and no marketed Foundayo tablet goes above 17.2 mg. The label reports the same trial program at 5.5, 9 and 17.2 mg. Do not convert between the two scales yourself.
Foundayo Price by Strength (Checked October 29, 2026)
As of October 29, 2026, LillyDirect and TrumpRx.gov list the same self-pay prices for a 30-tablet bottle (Lilly lists them as starting-at prices per 30-day month). Two are temporary.
Self-pay price per 30 tablets, checked October 29, 2026. The 2.5 mg and 5.5 mg reductions run through December 31, 2026.
| Strength | Price as of Oct 29, 2026 | Regular price | Per day, as of Oct 29, 2026 |
|---|---|---|---|
| 0.8 mg | $149 | $149 | $4.97 |
| 2.5 mg | $149 | $199 | $4.97 |
| 5.5 mg | $199 | $299 | $6.63 |
| 9 mg | $299 | $299 | $9.97 |
| 14.5 mg | $299 (Lilly Self-Pay Journey Program, refills within 45 days) | $349 (Lilly's regular price for a refill more than 45 days after the previous delivery; TrumpRx also lists $349) | $9.97 on time / $11.63 at $349 |
| 17.2 mg | $299 (Lilly Self-Pay Journey Program, refills within 45 days) | $349 (Lilly's regular price for a refill more than 45 days after the previous delivery; TrumpRx also lists $349) | $9.97 on time / $11.63 at $349 |
Assumptions: cash self-pay prices, not insurance or Medicare; taxes and fees excluded; per day is the bottle price divided by 30. Regular prices for 2.5 mg ($199) and 5.5 mg ($299) appear in Lilly's program terms and on TrumpRx, and apply once the reduction ends (it runs through December 31, 2026; Lilly says it may change or cancel the offer at any time). The $349 price for a refill more than 45 days after the previous delivery applies to 14.5 and 17.2 mg only (Lilly's program terms; TrumpRx).
The 45-Day Refill Rule for 14.5 mg and 17.2 mg
Lilly prices these two strengths through its Foundayo Self-Pay Journey Program at $299 a month. Its program terms say the first purchase at these strengths gets the $299 offer, each refill must be bought within 45 days of the previous delivery to keep it, and the regular price is $349 a month. TrumpRx lists the same $349 for a refill more than 45 days after your last delivery.
What the Fastest Path Costs
On the earliest possible calendar (one 30-tablet bottle per step, days 1 to 180, a step up at every first chance; each bottle is the first fill at its strength), six bottles cost $1,394 at October 29, 2026 prices, or $1,544 if the 2.5 mg and 5.5 mg reductions have ended. That is the fastest path's cost, assuming prices never change; any one person's bill depends on whether, and how quickly, the dose goes up.
Insurance and Medicare
Lilly says eligible people whose commercial insurance covers Foundayo could pay as low as $25 a month (government beneficiaries excluded; terms apply). Separately, Medicare.gov lists Foundayo as covered under the temporary Medicare GLP-1 Bridge from July 1, 2026, with a $50 monthly copay for eligible Part D enrollees; CMS says the program runs through December 31, 2027. Eligibility rules are in our guide to insurance and the Medicare GLP-1 Bridge and our Medicare GLP-1 Bridge eligibility checker.
Switching to Foundayo From Wegovy, Zepbound or Another GLP-1
The Foundayo label does not give a switching schedule. It does not mention Wegovy, Zepbound, semaglutide or tirzepatide, and it lists one starting dose for everyone: 0.8 mg once daily. It also says using Foundayo together with another GLP-1 receptor agonist is not recommended. For the two weight-management tablets side by side, label by label, see Foundayo vs Wegovy pill.
So this page has no conversion table and no equivalent dose for moving from an injection to a tablet. Starting above 0.8 mg would be a prescriber's decision outside the label. This page covers the label only; ask your prescriber about any switching evidence published elsewhere.
Safety Essentials Before You Dose
Boxed warning: risk of thyroid C-cell tumors. Such tumors appeared in rodents given GLP-1 drugs that are active in rats and mice; orforglipron is not active in rats or mice and did not cause tumors in rodents, and human relevance has not been determined. Foundayo is contraindicated with a personal or family history of medullary thyroid carcinoma (MTC) or with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Section 4 also contraindicates Foundayo for anyone with a known serious hypersensitivity (allergic reaction) to orforglipron or its ingredients. The label's warnings also include acute pancreatitis, severe stomach and bowel reactions, and kidney injury from dehydration after vomiting or diarrhea. For an overdose, the label points to Poison Control at 1-800-222-1222. For the full background on the drug, see our explainer on Foundayo and oral GLP-1s.
Frequently Asked Questions
What are the Foundayo doses?
Foundayo tablets come in six strengths: 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg. Treatment starts at 0.8 mg once daily and the maximum is 17.2 mg once daily. The label allows one tablet a day, swallowed whole, with or without food (Foundayo prescribing information, Section 2.1).
What is the Foundayo starting dose?
The label starting dose is 0.8 mg by mouth once daily. After at least 30 days on 0.8 mg the dose increases to 2.5 mg, and after at least 30 days on 2.5 mg it increases to 5.5 mg. Your prescriber decides when each step happens.
How long do you stay on each Foundayo dose?
At least 30 days on each dose before moving up, with no fixed pace after that. Increases to 9, 14.5 or 17.2 mg are based on treatment response and tolerability, and the label sets no maximum time on any step. The label permits, but does not require, increases above 5.5 mg.
How fast can you reach the 17.2 mg dose?
Counting the 30-day minimum at each of the five earlier steps, the earliest possible start for 17.2 mg is day 151, about five months in. That assumes no missed doses and a step up at every first chance. The label does not require reaching 17.2 mg at all.
Is there a Foundayo maintenance dose?
The label does not name a separate maintenance dose. It describes a starting dose, step increases after at least 30 days based on response and tolerability, and a maximum of 17.2 mg once daily. Which strength a person stays on is a decision made with their prescriber.
Can you take Foundayo with food, and at any time of day?
Yes on food: the label says once daily, with or without food, with the tablet swallowed whole and not broken, crushed or chewed. On timing, Lilly's patient website says Foundayo can be taken any time of day, with no planning around food or drink.
What if you miss a Foundayo dose?
Take it as soon as possible and do not double up the next dose. If 7 or more doses in a row are missed, the label says to restart dose escalation at a lower dose to reduce stomach side effects. It does not name which dose, so call your prescriber to plan the restart.
How much does Foundayo cost for each dose?
As of October 29, 2026, self-pay prices for a 30-tablet bottle are $149 for 0.8 mg and 2.5 mg, $199 for 5.5 mg, and $299 for 9, 14.5 and 17.2 mg. The 2.5 mg and 5.5 mg prices are temporarily reduced through December 31, 2026.
Can you switch from Wegovy or Zepbound to Foundayo, and at what dose?
The Foundayo label does not give a switching schedule and does not mention Wegovy or Zepbound. It lists one starting dose, 0.8 mg once daily, and says using another GLP-1 receptor agonist at the same time is not recommended. The switching plan is for your prescriber to decide.
Does Foundayo affect birth control pills?
It may. The label says Foundayo may reduce how well oral hormonal contraceptives work. It advises switching to a non-oral method, or adding a barrier method, for 30 days after starting Foundayo and for 30 days after each dose increase. Non-oral hormonal methods should not be affected.
Is there a Foundayo dose higher than 17.2 mg?
No. The label maximum is 17.2 mg once daily. Higher figures such as 36 mg come from the investigational tablet in the published ATTAIN-1 trial, not from a marketed Foundayo strength. Never take more than one tablet a day.
Comparing GLP-1 Costs and Schedules?
See cash prices for every GLP-1 on TrumpRx.gov. The free GLP3Planner calculator plots injectable GLP-1 schedules; it does not model Foundayo tablets.
